Piggyback Foundation Application

Eligibility Requirements

  1. Applicant must have a life-threatening illness and be a member of a family with children attending school (under the age of 22).
  2. Applicant must provide a letter from his/her treating physician documenting the type of illness diagnosed, the treatment prescribed, and a statement that the applicant is currently undergoing treatment.
  3. Recipients may receive funds for one year at the discretion of the board. If the treatment extends beyond one year from date of application, or if the treatment becomes necessary at a later date, the recipient may reapply at that time. To reapply, the applicant needs to submit a letter from the trating physician stating that treatment is still in progree or has started again.

Programs

Family Counts:

It is important to provide families the opportunity to attend activities that help strengthen the family unit, provide stress relief and make memorable moments. The Piggyback Foundation provides funding to families, at a time when finances are limited, to make memorable moments happen.

Birthdays Count sends birthday wishes, gift cards and provides funds for gifts and parties to all children in the family.

Education Counts:

The emotional stress and isolation felt by children, when attention has been directed to the ill family member, may cause them to fall behind in school. It is important to work with the parents, the school, and the teacher to meet the child's academic needs. The foundation provides funding for school fees, book fees, tutorig and educational materials.

Piggyback Readers provides support materials to help families cope and books for children and their families to enjoy together.

Kids Activities:

The Piggyback Foundation works with the family to maintain the children's extracurricular activities. In times of financial uncertainty, many families have to set priorities. The difficult decision to discontinue extracurricular activities can be devastating to a child's emotional, physical, and social well being. It is important that the children are able to continue to be involved in these enjoyable activities that help reduce stress.

Faith Counts:

If desired, we will support the family in prayer and faith development. The foundation believes that with God all things are possible.

Soup for Souls volunteers organize comfort baskets with homemade soup and snacks with prayers for healing for our PBF families.

Cards of Hope sends out messages of inspiration to those in need.

Emergency Fund:

Meeting the needs of emergency situations helps children and families feel more secure in their environment. The Piggyback Foundation works with the family to reduce financial stress by means of gas cards, grocery cards and other immediate needs.
